Job Summary
As a CenterWell Home Health LPN, provide skilled nursing care to patients in their homes under the direction of an RN, implementing individualized care plans, educating patients and families on disease management and self-care, and maintaining accurate records of visits and outcomes. You will coordinate with physicians and the healthcare team, participate in discharge planning, and maintain proactive communication regarding patient status. This role emphasizes one-on-one home health care, driving to patient residences, adherence to TB screening and driver-safety requirements, and contributing to high-quality in-home care across the care team.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Nursing experience in Home Health or Hospice setting (preferred)
- TB screening via Humana TB program
- Driver safety program participation
