Job Summary
Provide skilled nursing care to patients in their homes under the direction of an RN. Implement individualized home-care plans, educate patients and families on disease management and self-care, maintain accurate care documentation, and coordinate with physicians and the healthcare team. Participate in care coordination and discharge planning to support patient recovery and independence.
Required Qualifications
- Current nursing license in the practicing state
- Valid driver’s license
- Auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
