Job Summary
CenterWell Home Health is seeking a Home Health Licensed Practical Nurse (LPN) to provide skilled, one-on-one nursing care in patients’ homes under direction of an RN. Responsibilities include implementing individualized care plans in collaboration with patients and families, educating patients on disease management and self-care, maintaining accurate visit documentation, coordinating with physicians and the healthcare team, and participating in discharge planning. Requires an active LPN license in the practicing state, valid driver’s license with automobile insurance, CPR certification, and at least two years of LPN/LVN experience in a clinical setting; driving is required to perform home visits.
Required Qualifications
- Current nursing license in the practicing state
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
