Job Summary
Provide skilled nursing care to patients in their homes under the direction of an RN; implement individualized care plans in collaboration with the patient, family, and healthcare team; educate patients and their families on disease management, treatment options, and self-care techniques; maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes; maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes; participate in care coordination activities and discharge planning.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
