Job Summary
Provide skilled nursing care to patients in their homes under RN direction while implementing individualized care plans. Educate patients and families on disease management and self-care techniques. Maintain accurate documentation of visits, interventions, and outcomes. Coordinate with physicians and the clinical team to manage care plans and support discharge planning. Maintain contact regarding patient status changes promptly. This role supports patients managing chronic conditions or recovering from injury, illness, or surgery in the comfort of their homes.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- Required to be screened for TB
