Job Summary
Provide skilled nursing care to patients in their homes under RN direction while implementing individualized care plans. Collaborate with patients, families, and the healthcare team to educate on disease management and self-care techniques. Maintain accurate documentation of visits, interventions, and outcomes. Participate in care coordination and discharge planning activities. Work onsite with physicians and clinicians to support patient recovery and independence. Requires current nursing license, valid driver's license, and CPR certification. Full-time 40-hour schedule with pay per visit/unit.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- Required to be screened for TB
