Home Health Licensed Practical Nurse

Provide skilled nursing care to patients in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team; educate patients and families on disease management and self-care techniques; maintain accurate records of patient care and timely communication with the healthcare team. Participate in care coordination activities and discharge planning. This role is patient-facing, requires a valid driver’s license and auto insurance, CPR certification, and at least two years of LPN/LVN experience in a clinical setting. Schedule is 32 hours per week; compensation ranges are provided as pay-per-visit/unit rate ($33.00-$47.00). TB screening is required as part of Humana’s TB program.
