Job Summary
Provide skilled nursing care to patients in their homes under RN direction, implementing individualized care plans, educating patients and families on disease management and self-care, maintaining accurate patient care documentation, and coordinating with physicians and the healthcare team. Required Qualifications
- Current nursing license in the practicing state
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
