Job Summary
Provide skilled nursing care to patients in their homes under the direction of an RN, implement individualized care plans, educate patients and families on disease management and self-care, maintain thorough documentation of patient visits and outcomes, coordinate with physicians and the healthcare team, and participate in discharge planning and care coordination. Requires active LPN/LVN license in the practicing state, valid driver's license with reliable transportation, CPR certification, and at least two years of clinical LPN/LVN experience. Home visits and one-on-one patient care are emphasized; role supports Humana CenterWell Home Health within a patient-facing team.
