Job Summary
Conduct home health visits to adult and geriatric clients in Boston, Dorchester, Hyde Park, Roslindale, and surrounding towns. Follow designated care plans while performing assigned duties including medication administration, wound care, treatments, and procedures. Monitor client conditions, report changes to the Clinical Manager, and execute doctor's orders with proper documentation via touch pad tablet. Manage case coordination and perform client assessments as necessary. Maintain a current Massachusetts LPN license and one year of clinical experience. Required Qualifications
- One year prior clinical experience as a Licensed Practical Nurse
- A current license as an LPN in Massachusetts
- Graduation from an accredited and approved nursing program
