Home Health Intake Nurse - Registered Nurse (RN)

Coordinate patient transitions from acute care to Home Health Services by reviewing admission orders, conducting prescreening assessments, and ensuring continuity through referrals and physician communication. Facilitate patient transitions to appropriate post-acute levels while acting as a liaison between Home Health Services, community staff, and insurance providers. Collect health data from assessments impacting home care plans and identify barriers to resolution or service quality issues. Review orders for appropriateness and coordinate treatment protocols in accordance with agency policies. Requires RN licensure and experience with terminally ill patients; Monday-Friday day shift with rotating weekends.
