Job Summary
Manage the end-to-end patient intake process by receiving and processing referrals from hospitals, physicians, and discharge planners. Verify patient insurance eligibility, obtain authorizations for home health services, and coordinate with clinical staff to ensure timely start-of-care. Maintain accurate patient records in the EMR/EHR system while communicating effectively with patients, families, and internal care teams. Track and follow up on pending referrals and authorizations to minimize delays in care, ensuring compliance with Medicare, Medicaid, and private insurance guidelines. Twice monthly, travel to other doctor's offices to pick up signed POC documents. Work onsite in the Sterling office 30 to 40 hours a week, five days a week.
Required Qualifications
- High school diploma or equivalent
- Required to work onsite in the Office in Sterling 30 to 40 hours a week 5 days a week
- Twice a month will need to make trips to other doctor's offices to pick up signed POC documents (mileage is paid)
Desired Qualifications
- Prior experience in home health intake, medical scheduling, or healthcare coordination
- Familiarity with Medicare/Medicaid home health regulations and insurance authorization processes
- Proficiency with EMR/EHR software and Microsoft Office Suite
- Strong organizational skills with the ability to manage multiple referrals simultaneously
- Excellent verbal and written communication skills

- Associate's degree in healthcare administration or related field
