Job Summary
Registered Nurse (RN) Case Manager for Home Health/Hospice seeking to create and manage personalized care plans for patients. Responsibilities include coordinating with colleagues to assess, plan, implement, or evaluate patient care plans; administering medications and monitoring for side effects; documenting changes in patient conditions; maintaining accurate records and communicating observations to the supervisor. Requires active RN license, nursing experience, valid driver’s license, and First Aid/CPR certification. Flexible scheduling and advancement opportunities available. Location: Murray, UT; Employment: Full-time.
Required Qualifications
- Currently licensed Registered Nurse (RN) in state of employment
- Experience as a Registered Nurse
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
