Home Health & Hospice Registered Nurse

Provide admission, evaluation, and follow-up skilled nursing visits for home health patients while delivering quality hands-on care, managing, and teaching patient care plans under Clinical Manager supervision. Coordinate care with the healthcare team including physical therapists, occupational therapists, home health aides, and medical social workers. Report patient progress to the physician and Clinical Manager to maintain continuity of care and implement the plan of care with the patient and family to achieve optimal outcomes. This role serves patients in Santa Clara and San Benito Counties with flexible scheduling options.
