Home Health & Hospice Registered Nurse

Provide admission, evaluation, and follow-up skilled nursing visits for home health patients. Deliver quality hands-on care, manage, and teach patient care plans in accordance with physician orders under Clinical Manager supervision. Coordinate care with Physical Therapists, Occupational Therapists, Home Health Aides, and Medical Social Workers. Report patient progress to the physician and Clinical Manager to maintain continuity of care. Implement the plan of care with the patient and family to assist them in achieving optimal outcomes.
