Home Health & Hospice Registered Nurse

Provide admission, evaluation, and follow-up skilled nursing visits for home health patients, delivering quality hands-on care and teaching within the patient's care plan under Clinical Manager supervision. Coordinate care with the multidisciplinary team including physical therapists, occupational therapists, and home health aides while reporting patient progress to the physician to maintain continuity. Implement the plan of care with the patient and family to assist in achieving optimal outcomes. Full-time employment with flexible scheduling available for Alameda County residents.
