Home Health & Hospice Registered Nurse

Provide admission, evaluation, and follow-up skilled nursing visits for home health patients while delivering quality hands-on care and teaching care plans per physician orders. Coordinate with the healthcare team including physical therapists, occupational therapists, and home health aides to implement the plan of care and report patient progress to maintain continuity. Assist patients and families in achieving optimal outcomes through personalized care in their homes. Full-time and part-time roles available with sign-on bonuses up to $5,000 for full-time RNs.
