Home Health & Hospice Registered Nurse

Provide admission, evaluation, and follow-up skilled nursing visits for home health patients while delivering quality hands-on care, managing, and teaching patient care plans under Clinical Manager supervision. Coordinate care with the broader healthcare team including Physical Therapists, Occupational Therapists, Home Health Aides, and Medical Social Workers. Report patient progress to physicians and Clinical Managers to maintain continuity of care and implement plans of care with patients and families to achieve optimal outcomes. This role supports Prohealth's patient-focused model of skilled, dignified care in patients' homes across Northern California, offering flexible scheduling and comprehensive training.
