Job Summary
Provide direct patient care under the supervision of a registered nurse by implementing the plan of care, delivering treatments, and assessing patients for education regarding their diagnosis and care plan. Collaborate with the multidisciplinary team, including home health aides, physical therapists, and social workers, to coordinate services and report identified needs to the clinical supervisor. Work flexible schedules across Santa Clara and San Benito County to deliver skilled, dignified care in patients' homes while supporting families.
Required Qualifications
- Valid LVN License to practice in CA
- Current BLS/CPR certification
- Valid CA Driver's license
Desired Qualifications
- Minimum one year Clinical experience as LVN/Home Health
