Provide direct patient care under the supervision of a registered nurse by implementing the plan of care, administering treatments, and assessing patients for diagnosis and care needs. Collaborate with the multidisciplinary team, including home health aides and therapists, to coordinate services and report identified patient needs to the clinical supervisor. Educate patients and families regarding their diagnosis and care plan while maintaining dignified home-based support. This role offers a flexible schedule allowing you to manage your own visits.
