Job Summary
Deliver one-on-one skilled nursing care in patients' homes as the primary clinical partner, including in-home assessments and tailored care plans. Provide wound care, infusions, catheter care, medication management, post-operative and chronic disease management. Educate and empower patients and caregivers to manage symptoms, medications, and daily care. Monitor progress, identify changes, coordinate with physicians and the interdisciplinary team, adjust plans to prevent hospitalizations, and complete timely documentation to support quality outcomes. Requires active RN license and CPR certification; valid driver's license and reliable transportation for home visits; on-call rotation included.
Required Qualifications
- Active Registered Nurse (RN) license in state of practice
- CPR certification (or ability to obtain)
- Valid driver's license, reliable transportation, and auto insurance
- 1 year of clinical nursing experience
