Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Provide written evaluations and plans of care, documenting client performance and modifying treatment as needed. Collaborate with physicians, nurses, and families on discharge planning and home exercise programs. Complete OASIS assessments and maintain effective communication with the healthcare team. Supervise PTAs and HHAs as applicable, ensuring all documentation meets payer standards to prevent denials.
