Home Health Field RN PRN

Home Health Field RN provides patient-centered, in-home care including comprehensive assessments, plan-of-care development and documentation, admission and re-certification decisions, and coordination with interdisciplinary teams. Responsibilities include education for patients and families on self-care, nutrition, and care needs; determining scope and frequency of services; initiating preventive and rehabilitative nursing procedures; applying infection control practices; supervising Home Health Aide and LPN/LVN to ensure continuity of service; and communicating changes in patient status to physicians and team members. Requires current RN license, valid driver’s license, CPR certification, valid auto insurance, and typically 2+ years of Home Health experience; HomeCare HomeBase experience preferred. Role is PRN/as-needed and focused on delivering high-quality home-based nursing care.
