Home Health Field RN

The Home Health RN provides patient-centered, in-home nursing care, conducting initial and ongoing assessments, developing and documenting plans of care, educating patients and families on self-care, and coordinating with interdisciplinary teams to ensure quality and continuity of home health services. Responsibilities include evaluating acuity to determine service levels, initiating preventative and rehabilitative nursing procedures, applying infection control practices, supervising and directing Home Health Aide and LPN/LVN staff, preparing progress notes, and communicating changes in patient needs to physicians and care teams to optimize outcomes and independence.
