Conduct comprehensive physical, functional, and psychosocial assessments for home health patients, applying admission and re-certification criteria to establish appropriate care levels. Implement and document individualized plans of care while initiating preventative and rehabilitative procedures, ensuring infection control and standard precautions are maintained. Provide clinical direction to Home Health Aides and LPNs/LVNs, educate patients and families on self-care techniques, and monitor cases for third-party payor compliance. Prepare clinical progress notes, communicate changes in patient needs to physicians, and evaluate caregiver abilities to meet immediate requirements.
