Job Summary
Provide home-based nursing care as a Home Health RN, performing comprehensive patient assessments, evaluating eligibility for services, developing and updating individualized plans of care, educating patients and families on disease management and safety, determining service frequency based on acuity, performing preventive and rehabilitative nursing interventions, adhering to infection prevention standards, supervising Home Health Aides and LPN/LVNs, documenting patient progress and outcomes, communicating changes to physicians and the interdisciplinary team, and ensuring payer requirements and quality standards are met.

- Valid Driver’s License
- Current CPR Certification
- Valid Auto Insurance
