Job Summary
Entry-level Field Physical Therapist (PT) in home health focusing on hands-on clinical interventions and home-based care. Provides gait training, balance and neuromuscular re-education, transfer training, therapeutic exercises, manual therapy, and pain management under supervision of a Physical Therapist Case Manager. Conducts detailed home evaluations and assessments, implements individualized plans of care, documents care within 24-48 hours, educates patients and caregivers, collaborates with the interdisciplinary team (RN, OT, ST, MSW, HHA), participates in care plan updates and weekly case conferences, maintains privacy and dignity in care delivery, and meets productivity expectations. Requires CA PT license, BLS/CPR, CA driver’s license, and willingness to work in the Vacaville/Novato area; on-call/weekend rotation may be required.
Required Qualifications
- Current, unrestricted California Physical Therapist (PT) License.
- Current BLS/CPR Certification.
- Valid CA Driver’s License with a clean driving record.
- Graduate of an accredited Physical Therapy program.
