Job Summary
Home Health Community Liaison responsible for building and maintaining relationships with referral partners (case managers, discharge planners, hospitals, nursing facilities, physicians), executing sales calls to meet referral needs, educating the medical community on home health services, and achieving monthly production goals. Collaborates in interdisciplinary team meetings, supports intake coordination and timely admissions, handles sales administration duties, and participates in marketing efforts. Requires an Associate Degree or equivalent, home health/hospice experience, strong communication skills, proficiency with CRM and standard office tools, ability to travel locally, and a valid driver's license. Job Type: Full-time. Work Location: In person in San Diego, CA.
Required Qualifications
- Associate Degree or equivalent required
- Previous Home Health or Hospice experience is mandatory
- Knowledge of medical terminology
- Must have excellent written and verbal communication skills
- Demonstrated strong customer service, and relationship-building skills
- Good computer skills including Email, CRM software, Google Workspace, Google Meet/Zoom/Video Conference, PowerPoint, MS Word, and MS Excel
- Must be able to travel with short notice as required for the above duties to clients and community locations
- Must have a car with current insurance coverage and a valid driver's license
