Job Summary
Deliver compassionate, patient-centered care in a hospice setting by assisting with activities of daily living and reporting condition changes to support patient safety. Educate patients and caregivers on care plans while responding promptly to needs and maintaining accurate EMR documentation. Work independently within an assigned territory, traveling to patient homes in the Thomasville, PA area. Requires an active CNA certificate, one year healthcare experience, and a current driver's license for a part-time role of 20 hours per week.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certificate in the state you work
- High School Diploma or certificate
- Minimum one year healthcare experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be legally authorized to work in the United States
