Home Health Clinical Social Worker (LCSW) – South

Conduct comprehensive psychosocial assessments, crisis intervention, and ongoing support for patients and families receiving skilled home health services in Southern Fairfield County. Develop individualized interventions addressing social, emotional, financial, and environmental barriers while coordinating referrals with community agencies. Collaborate with the interdisciplinary care team to facilitate care planning, advance directives, and goals of care discussions. Maintain compliance with Medicare, Medicaid, HIPAA, and agency policies while completing accurate electronic medical record documentation. Participate in case conferences, staff meetings, and continuing education programs. Full-time role with 30 hours per week.
