Home Health Clinical Manager

Supervise all patient care services and staff including RNs, LPNs, OTs, PTs, and HHAs while coordinating patient care, referrals, and ongoing assessments. Provide direct patient care when staffing requires, support clinicians with OASIS documentation and case management, and review clinical documentation to ensure charts remain accurate and regulatory compliant. Lead case conferences, clinical meetings, and in-services while monitoring clinical outcomes. Maintain compliance with individualized plans of care and agency standards.
