Home Health Clinical Manager

Lead clinical operations for the CenterWell Home Health location in Lake Mary, overseeing patient care delivery, staff management, documentation quality, and regulatory compliance onsite. Guide and educate clinicians, support care planning and clinical decision-making, ensure Plans of Care meet patient needs and agency standards, monitor Medicare/payor documentation and billing, participate in hiring, training, performance evaluation, coaching, and corrective action, drive quality improvement initiatives, coordinate communication among physicians, team members, and caregivers, and support care coordination, discharge planning, and outcomes. May provide direct patient care in exceptional circumstances and act as Branch Director in their absence. Requires active RN license, CPR, valid driver's license and reliable transportation, and Home Health experience with management experience in a home care/hospice setting preferred. Schedule includes 40 hours per week with compensation range $77,200–$106,200; eligible for incentive bonus. The role is patient-facing and part of Humana CenterWell Home Health.”,
