Job Summary
Conduct facility visits at physicians' request to assist the clinical team in determining eligibility for home health services. Maintain contact with key hospital, skilled nursing, and assisted living facility discharge planning services to provide ongoing updates on available Company services. Ensure effective communication and collaboration with program staff and field sales resources via weekly meetings. Assist the program in processing physician orders as directed. Travel within an assigned territory and attend sales meetings as required. This role is considered patient-facing and is part of Humana's Tuberculosis screening program.
Required Qualifications
- RN/LPN/LVN license
- Must have strong knowledge of governmental regulations
- Must have knowledge of Medicare eligibility requirements
- Must have comprehensive understanding of potential care plan needs for the patient
- Must have understanding of coordination of necessary resources
- Must be able to travel within an assigned territory
- Must be able to travel to sales meetings as required
- Must be screened for TB
