Job Summary
Deliver compassionate, patient-centered care in a hospice setting by assisting with activities of daily living and observing changes in condition to support patient safety. Educate patients and caregivers on care plans while maintaining accurate EMR documentation and communicating effectively with the interdisciplinary team. Respond promptly to patient needs and travel to assigned territories within Pottsville, PA, spending approximately 20% of your day driving. Work independently but never alone in this part-time role (20 hours/week) for Residential Home Health and Hospice, a leader in home-based healthcare for geriatric and chronically ill patients.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certificate in the state you work
- High School Diploma or certificate
- Minimum one year healthcare experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
