Job Summary
Deliver compassionate, patient-centered care in a hospice setting by assisting with activities of daily living and observing changes in condition to support patient safety. Educate and support patients and caregivers by reinforcing care plans and promoting healthy routines. Respond promptly to patient needs, communicate effectively with the interdisciplinary team, and maintain accurate, timely documentation in the EMR. Work independently within assigned territory while traveling to patient homes, spending approximately 20% of your day driving. Requires an active Certified Nursing Assistant certificate, high school diploma, minimum one year healthcare experience, and a current driver's license.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certificate in the state you work
- High School Diploma or certificate
- Minimum one year healthcare experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be legally authorized to work in the United States
