Provide in-home speech therapy services for home health patients on Oahu, perform evaluations and develop individualized treatment plans, address swallowing disorders and cognitive-communication deficits, collaborate with nurses, therapists, and case managers, and maintain Medicare-era documentation in a home-health setting.
Required Qualifications
- Hawaii license as Speech-Language Pathologist (SLP)
- 1+ year clinical experience (home health experience a plus)
- Strong documentation and communication skills
- Ability to work independently in the field
- Valid driver’s license + reliable transportation
