Home Health Care Patient Coordinator

Job Summary

Serve as the primary point of contact for patients, families, and caregivers, addressing inquiries with empathy. Conduct initial patient intake to gather medical information and determine care plans. Coordinate and schedule appointments with nurses and therapists while maintaining accurate records in the EHR system. Process paperwork including referrals and insurance documentation, and monitor patient progress to escalate changes to clinical staff. Assist with discharge planning and resolve scheduling conflicts promptly. Educate families on services and insurance coverage while adhering to HIPAA regulations. Join team meetings to contribute to continuous quality improvement initiatives.

Required Qualifications

  • High School Diploma or GED
  • Associate's or Bachelor's degree in healthcare administration, nursing, or a related field
  • Minimum of 2 years of experience in a healthcare coordination, patient services, or administrative role
