Job Summary
Conduct 12–15 daily in-person stops at hospitals, physician offices, skilled nursing facilities, and assisted living communities to build referral relationships and obtain physician orders. Visit referral sources face-to-face to introduce services as a Medicare-certified home health agency, educate clinical teams, and coordinate patient transitions into home care. Execute a structured territory plan, track all activity in CRM, and consistently hit daily stop targets to grow your market presence. Full-time roles include Monday through Friday, 8:30 AM to 5:00 PM, with uncapped performance bonuses.
Required Qualifications
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver's license and reliable transportation
