Generate patient referrals from hospitals, skilled nursing facilities, physician offices, and assisted living communities by conducting 12–15 in-person stops daily. Build face-to-face relationships with referral sources, follow up on patients, and obtain physician orders to coordinate smooth transitions into home health services. Educate clinical teams on Medicare-certified home health offerings while executing a structured territory plan and tracking all activity in CRM. This field-based sales role requires consistent visibility and reliability to grow your market share month over month.
Required Qualifications
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver's license and reliable transportation
