Job Summary
Conduct 12–15 daily in-person stops at hospitals, physician offices, skilled nursing facilities, and assisted living communities to build referral relationships and obtain physician orders. Educate clinical staff on Medicare-certified home health services, coordinate patient transitions, and follow up on referrals while executing a structured territory plan. Track all activity and referrals in CRM systems and represent the company professionally across multiple states. This field-based role requires consistent visibility, reliable transportation, and the ability to drive business growth month over month.
Required Qualifications
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver's license and reliable transportation
