Job Summary
Conduct 12–15 in-person daily stops at hospitals, physician offices, skilled nursing facilities, and assisted living communities to build relationships and generate patient referrals. Introduce and maintain connections with referral sources while following up on patients and obtaining physician orders for home health admissions. Educate clinical teams on Medicare-certified services, coordinate patient transitions, and track all activity in CRM. Execute a structured territory plan to ensure consistent visibility and reliable relationship growth across your market.
Required Qualifications
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver's license and reliable transportation
