Home Health Care Marketer / Liaison (Field-Based Sales Role)

Generate patient referrals from hospitals, skilled nursing facilities, physician offices, and assisted living communities by conducting 12–15 in-person stops daily. Build and maintain strong referral relationships while obtaining physician orders and required documentation to coordinate patient transitions into home health services. Educate clinical teams on Medicare-certified home health services and execute a structured daily territory plan. Track all activity and referrals in CRM systems and represent the company professionally at all times. This field-based role requires a self-motivated sales professional comfortable working independently across multiple states.
