Job Summary
Conduct 12–15 daily in-person stops at hospitals, physician offices, skilled nursing facilities, and assisted living communities to build relationships and generate patient referrals. Introduce services as a Medicare-certified home health agency, follow up on referrals, and obtain physician orders while tracking all activity in CRM. Coordinate patient transitions into home health services and communicate with clinical teams to ensure smooth admissions. Execute a structured daily and weekly territory plan, representing the company professionally across multiple states.
Required Qualifications
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver's license and reliable transportation
