Job Summary
Conduct 12–15 daily in-person stops at hospitals, physician offices, skilled nursing facilities, and assisted living communities to build relationships, obtain referrals, and coordinate patient transitions into home health services. Educate clinical staff on Medicare-certified services, follow up on leads, and secure physician orders while executing a structured territory plan. Track all activity and referrals in CRM daily. Represent the company professionally across multiple states. Full-time positions require Monday through Friday hours with a valid driver's license; part-time roles offer flexible schedules within business hours.
Required Qualifications
- Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar
- Strong communication and relationship-building skills
- Ability to work independently and manage a territory
- Valid driver's license and reliable transportation
