Job Summary
Develop and maintain referral relationships with healthcare providers and community partners; Promote Choice Health at Home as the preferred provider for home health services; Identify new business opportunities and referral sources; Collaborate with clinical and operational teams to support patient admissions and care transitions; Educate referral partners on services, programs, and patient eligibility; Ensure compliance with all federal, state, and local marketing regulations; Participate in community outreach, networking events, and professional organizations; Meet and exceed monthly admission and growth goals; Provide market feedback and recommendations for service expansion.
Required Qualifications
- 2+ years of home health or hospice sales experience preferred
- Strong relationship-building and communication skills
- Experience using CRM systems preferred
- HomeCare HomeBase experience preferred
- Valid driver's license and auto insurance required
