Job Summary
Conduct initial and ongoing comprehensive assessments to determine home care needs, including physical and behavioral health evaluations. Develop, implement, and adjust care plans based on patient status and nursing diagnosis. Administer prescribed medications and treatments, provide therapeutic and rehabilitative nursing interventions, and educate patients and families on health management, preventive care, and recovery strategies. Collaborate with physicians, specialists, and community healthcare professionals to ensure seamless, holistic patient care. Communicate updates regarding patient condition and secure necessary physician orders. Prepare and maintain accurate clinical records, ensuring timely updates for physicians and compliance with healthcare regulations. Participate in on-call duties as required. Coordinate necessary medical equipment and services to enhance patient comfort and recovery. This role requires a graduate of an accredited school of nursing with an active state RN license and minimum of one year of recent RN psychiatric experience. Pay range is $65-70/hr. Schedule is part-time, Monday-Friday varying days as needed. Location is North San Diego county.
Required Qualifications
- Graduate of an accredited school of nursing
- Active state RN license
- Minimum of 1 year of recent (within the last 3 years) RN psychiatric experience
- Valid driver's license
- Auto insurance
