Job Summary
Provide skilled nursing care onsite in patients' homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans in collaboration with physicians, monitor conditions, and report progress to the Clinical Manager. Educate patients and families on disease management while maintaining accurate records and overseeing Home Health Aides. Requires current RN licensure, med surg/ICU experience, and a valid driver's license. Pay ranges from $77,200 to $106,200 annually for 40 hours per week.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
