As a Home Health Physical Therapist at CenterWell Home Health, you will plan and administer prescribed physical therapy treatment for patients in their homes to maximize physical function, assess and document patients’ physical strengths, develop and implement rehabilitation programs aligned with physician plans, provide clear instructions to PTAs and Home Health Aides, document observations, and report progress to physicians and care teams. The role requires collaborating with healthcare professionals, driving to patient homes, and meeting 40 hours per week with a pay range of $93,000-$128,000 annually. TB screening and a valid driver’s license with adequate auto insurance are required. A current and unrestricted Physical Therapy license and CPR certification are required, with one year of experience in physical therapy preferred.
