Job Summary
Home Health and Hospice Social Worker provides medical social work services in home health and hospice settings. Responsibilities include psychosocial assessments of patients and communication of findings to the RN, conducting social evaluations and planning interventions, maintaining clinical records, providing information/referral services to patients, families, and community agencies, serving as a liaison with community agencies, fostering collaborative relationships with organization staff, developing contracts with agencies, participating in the total plan of care and case conferences, assisting the care team in understanding social and emotional factors affecting health, and participating in discharge planning. Other duties as delegated by nursing leadership.
Required Qualifications
- Bachelor's or Master’s degree in social work accredited by the Council on Social Work Education (CSWE)
- MSW preferred
- Minimum of one year's experience in health care
- Arizona State Board of Behavioral Health Examiners licensure preferred (AZBBHE)
- CPR certification current
- Must be a licensed driver with an automobile insured and in good working order
- Experience in home health or hospice care preferred
