Job Summary
Conduct skilled nursing care for Home Health and Hospice patients requiring intermittent services from 5pm Friday through 8am Monday, including weekend call assignments. Administer treatments, develop and implement patient care plans in MatrixCare, and educate families on disease processes and end-of-life care. Manage assigned cases by observing symptoms, reporting changes to physicians, and supervising Nursing Assistants with written plans. Collaborate with supervisors to meet productivity standards, ensure timely paperwork submission by Monday 5pm, and participate in quality improvement activities and in-services. Maintain compliance with agency policies while striving to improve nursing care through continuing education and professional organization involvement.
Required Qualifications
- Registered Nurse currently licensed to practice nursing in the state of North Carolina
- Minimum of one-year experience as a professional nurse in an acute care setting
- Current CPR certification (BCLS)
- Basic computer skills/knowledge and experience in electronic medical records
- Ability to understand and communicate well with patients, family, staff, physicians and the public
- Dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
- Competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized a plan of care
- Valid Driver's License and reliable transportation
- Must be able to work from 5pm Friday thru 8am Monday
- Must be able to take call as assigned during these hours
- Must be able to see scheduled visits after 5:00 p.m. on Fridays
- Must work at least one weekend day when Christmas falls on a weekend
- Must be willing to take weekend call as assigned
- Must attend the appropriate hours of in-service training per year as required
- Must adhere to the Agency's personnel policies and Compliance Plan
- Must be willing to perform other duties as assigned by supervisor
- Must ensure referrals are evaluated timely with a NPU goal of 6% or less
- Must assist with covering visits in other offices as assigned
- Must display a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors
- Must be able to obtain a medical history from the patient and / or family member (s)
- Must be able to perform a physical assessment of the patient
- Must be able to assess the patient's support systems
- Must be able to assess the patient, family member (s) and home situation
- Must be able to interpret nursing and other services of the Agency to patient's and families
- Must be able to develop and implement the patient care plan
- Must be able to manage assigned cases and provide skilled nursing care
- Must be able to observe signs and symptoms and report to the physician
- Must be able to adequately educate, teach, supervise, consult, and counsel the patient / family members
- Must be able to initiate appropriate preventative and rehabilitative nursing procedures
- Must be able to ensure that care is provided by the Home Health Nursing Assistant
- Must be able to appropriately consult with the attending or on-call physician
- Must be able to communicate with supervisor concerning problems of the patient(s)
- Must be able to turn in all paperwork to the designated person in the office by 5pm on Monday
- Must be able to sync ipad nightly when visits are done
- Must be able to make a verbal report of new admissions and ER visits to the primary nurse Sunday night or Monday morning between 8:00-9:00 a.m.
- Must be able to collaborate with supervisor to meet assigned productivity standards
- Must be able to keep up-to-date on reimbursement criteria and charting needs for all patients under his/her care and supervision
- Must be able to attend and participate in office staff meetings, patient care conferences, record audits, utilization review, quality improvement/assurance activities, in-services, workshops, and seminars when appropriate
- Must be able to continually strive to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings
- Must be able to evaluate own needs for support and use identified system(s) to meet those needs
